At Crystal Coast Oral & Facial Surgery, we make every effort to provide not only the finest surgical care, but convenient financial options. Payment is due at the time service is rendered unless other arrangements have been made. Some procedures may require payment in advance. We accept Visa and MasterCard, and for patients that will benefit from a monthly payment plan, we can assist you in making arrangements through Health Care Credit Line prior to your treatment. We will make every reasonable attempt to maximize your insurance benefits. Please be aware, however, that any insurance coverage you have is a contract between you and the insurance company. You are fully responsible for any balance due on services you have received.

Although payment is due on the day of service, some patients may have a balance after treatment. Sometimes this balance cannot be calculated accurately until your insurance carrier has processed the insurance claim. If you still have a balance after the insurance has paid their portion, we will send a statement to you. Your prompt payment is appreciated.

For those patients needing procedures that require a hospital-type setting, the surgery can be completed at one of the local hospitals or at an outpatient surgical center. Dr. Hagan is credentialed at Westmoreland and Frick Hospitals and Laurel Surgical Center in cosmetic facial surgery.

Important: You and your insurance carrier are responsible for any charges related to procedures conducted outside of our office that may come in the form of hospital and anesthesia charges. These will be billed separately by the service providers.
